Warner Place, medical model adult day healthcare program, is named in honor of the Warner Home Family. This beautifully designed building opened in July, 2010 to serve those who need nursing home care during the day and have a support system so they can remain at home. The goal is to allow people to stay independent longer and offer respite for caregivers.

We know that no one can replace the love and compassion you give as a primary caregiver. We are here to support you and to be your partner in providing the very best experience for your loved one. This is a ‘win/win’ situation for everyone in the family. Caregivers receive the support and respite they need, while the person attending the program can continue to live at home and benefit from the caring staff and all the services provided.
Benefits for Caregivers
- We offer a safe environment, where your loved one can move about independently, under direct supervision.
- We can assist with bathing and hygiene.
- We monitor nutritional status and provide nutritious snacks and meals.
- We work hand in hand with each caregiver and participant to create a customized plan of care for your loved one, including medication management and treatments.
- Our activities are based on the likes and dislikes of each participant. We are open to ideas and suggestions.
- While your loved one is with us, you have the flexibility to do the things you need to do, including taking some time for yourself and other members of your family.
- We work closely with other members of the healthcare team, such as the primary care doctor, dietitian and therapists.
- You are welcome to stop by and visit anytime you wish.
- You’ll have peace of mind knowing that we provide friendship and socialization, in a warm and welcoming atmosphere.
