Warner Place Medical Model Adult DayCare Program
Warner Place on the Lutheran Campus in Jamestown is the area’s only medical-model adult daycare and healthcare program for adults. The program is designed for individuals who require skilled nursing care during the day but can remain at home with their family or caregivers in the evenings. The goal of Warner Place is to keep individuals at home and out of a long-term care facility as long as possible. We know that no one can substitute the love and compassion you give as a primary caregiver. That is why we are your partner in the caregiving process, not the replacement.
Warner Place can provide caregivers the support, flexibility, and respite they need to care for themselves and other family members with a peace of mind knowing their loved one is being cared for appropriately.

Who can register at Warner Place?
Warner Place serves a diverse population of individuals with functional limitations and provides a peace of mind for their caregivers and loved ones. Warner Place is the perfect option for individuals requiring assistance for at least five (5) hours a day, one (1) day per week, for thirty (30) or more days.
Prior to admittance, Warner Place registrants will go through a needs assessment to determine their individualized care plan. These care plans will determine the services provided for them by the staff at Warner Place which consists of: an RN, LPN, CNA, COTA, and many other professionals to monitor all medical conditions and social programming for daily registrants we take into consideration everyone’s interests and abilities.
How do we pay for services at Warner Place?
Individuals interested in enrolling their loved ones in the Warner Place program must be Medicaid eligible, be willing or able to pay privately, or have a private form of insurance that will cover the service. Medicare does not currently pay for adult day health care services but coming soon Medicare Advantage Plans will be covering them.
Is transportation provided?
Although transportation to and from Warner Place is not provided directly by our staff, we do help plan and coordinate transportation through a number of local services in the area to ensure your loved ones here and back safely all year round.
How do I start the registration process?
First, you should call Warner Place to schedule a consultation and tour of the facility. After meeting with our staff, you should contact your loved one’s physician for a referral to Warner Place.
